Summer-camp season
Camp asks for a health and medical record and, often, a provider-signed physical. Collect them ahead of check-in so nobody is turned away at the gate.
The paperwork a troop already runs — e-signed or collected in one flow:
- Trip permission slip
- Your activity consent for the campout, hike, or service event — the parent e-signs it for their scout.
- Health & medical form
- Your annual health record (for example an AHMR Part A/B). A family's saved profile autofills the medical and emergency details, so they aren't rewriting it every trip.
- Provider-signed physical
- A physical a clinician signs offline (like an AHMR Part C) isn't e-signed here — mark it required and the parent uploads the completed form. ClearedFor collects and stores it for your leaders; it does not verify the provider or the contents.
- Photo / media release
- Add a media release if your council or chartered organization asks for one. Signed once, it clears with the rest of the set.
- The org
