Trip permission slip

Your activity consent for the campout, hike, or service event — the parent e-signs it for their scout.

Health & medical form

Your annual health record (for example an AHMR Part A/B). A family's saved profile autofills the medical and emergency details, so they aren't rewriting it every trip.

Provider-signed physical

A physical a clinician signs offline (like an AHMR Part C) isn't e-signed here — mark it required and the parent uploads the completed form. ClearedFor collects and stores it for your leaders; it does not verify the provider or the contents.

Photo / media release

Add a media release if your council or chartered organization asks for one. Signed once, it clears with the rest of the set.

The org

Each troop, pack, or crew is its own organization. Adult leaders are leaders; each scout's parent or guardian signs.

An activity

A campout, a hike, a jamboree, or a service event — one activity per outing, or one for a whole season of standing forms.

Required set

The set you require: the permission slip and health form to e-sign, plus any provider physical you collect. A scout is cleared once every required item is done.
